This section is used to CREATE a NEW account for a facility seeking Substance Use Disorder Treatment Center Accreditation from CIHQ.
If your facility already has Substance Use Disorder Treatment Center Accreditation by CIHQ and you wish to make changes to your application please login to your existing account.
If you’re a facility employee seeking WEBSITE ACCESS please contact your supervisor for your username & password.
How to Start the Process
Step One: We Need to Establish an Account for You
In order for your facility to be certified, you will need to complete a simple application. To do so, we must first create an account for you.
Once we have established your account, you will receive an email from us entitled: "Your SUDTC Account". Please save this email for future reference. Check your spam filters or junk mail in case your facility's email system does not recognize us. If you do not receive an email from us within 48 hours, please contact Traci Curtis at tcurtis@CIHQ.org for assistance.
Step Two: Log Into Your Account and Complete Your Application
Login to your account, complete and submit your application for certification.
